Register with the Center for the Blind

Registration is required before you can receive our resources and services. There are two ways to register.

1. Online Registration: Fill-in our online registration form for the quickest registration.

2. Postal Mail Registration: Download our registration form (PDF). Print out the registration form, fill in the requested information and mail the completed and signed registration form to the address on the form.
